
The American College of Cardiology (ACC) and Evidation Health are joining forces to develop Achievement for Heart Health. This first-of-its-kind health engagement experience will deliver evidence-based content, resources, and tools to thousands of individuals experiencing heart failure. This program, built on Evidation’s Achievement platform, will make it easier than ever before for individuals to better understand and engage with their cardiovascular health outside clinic-based settings.
The Link Between Lifestyle Habits and Heart Health
An intensive lifestyle intervention promotes achievement of important heart health characteristics that, if maintained, may substantially reduce CVD events. Initially focusing on heart failure, Achievement for Heart Health is a curated health program that equips individuals to better manage their heart health from anywhere. Following the personalized care model, those enrolled in the program can continuously monitor and learn from data relative to their cardiovascular health from the convenience of one’s home. Individuals can share activity, sleep, blood pressure, and symptom information, including wearable data. By sharing this physiologic data, participants will receive personalized, evidence-based educational content on topics like heart medications, nutrition, and stress management from CardioSmart, ACC’s patient engagement program, including a library of cardiovascular health resources, as well as patient navigation tools to guide provider visits.
Social Detriments of Health (SDOH)
Research has found that individual behavior, often driven by social determinants, accounts for up to 40 percent of the risk of premature death. These findings helped establish the Commission on Social Determinants of Health by the World Health Organization (WHO) in 2005. The Commission’s responsibilities require that they assemble, collate, and synthesize global data on SDOH and its impact on health inequity, and to make recommendations for action to address that inequity. The goal of the Commission is simple: to foster a global movement on SDOH and health equity. The Commission’s first report in 2006 stated ‘the link between people’s health and their surrounding can no longer be ignored.
Since 2018, health systems in the U.S. have publicly committed approximately $2.5 billion toward directly addressing SDOH such as job training, food security, and housing. Once we can all agree on this, it is then necessary to develop a digital infrastructure that is interoperable for the integration of social care into the healthcare delivery model. This will also require a workforce that’s able to integrate social care into health care delivery.
SDOH is much more complex than simply rich versus poor. Social determinant factors have a significant impact on how patients engage with their health. It is vital to remember that SDOH is not the result of any natural law nor do they exist in a vacuum. Health equity is an issue within all countries worldwide and is influenced significantly by the global economic and political system. Within the U.S., the design, implementation, and evaluation of effective policies at the local, state, and federal levels to improve health outcomes can be improved through a deeper understanding of the complexities of social and economic disparities.
Collaborating with Achievement of Heart Health
The Achievement for Heart Health program will be open to partners following Evidation’s consent-per-use model, where participants grant permission for each program in which they participate and receive compensation for their contribution to research. Achievement, Evidation’s app and network of nearly 4 million individuals, is the largest and most demographically and geographically diverse connected group in the United States, representing 50 states and 9 out of every 10 ZIP codes nationwide.
The Achievement for Heart Health program will help ACC and our research partners better understand the complex, individualized patient journey of those with heart failure, with a goal of improving how patients live with and manage one of the most common cardiovascular conditions affecting Americans today. The combination of Evidation’s platform and ACC’s clinical and scientific expertise will enable groundbreaking opportunities to transform cardiovascular care and improve heart health and outcomes.
